How we may use and disclose information
SCCC may use or disclose protected health information for treatment, payment, health care operations, benefits verification, claims, required reporting, and other uses permitted or required by law.
Your rights
You may request access to your records, request corrections, request restrictions, request confidential communications, receive an accounting of certain disclosures, and request a copy of this notice.
Our duties
SCCC is required by law to maintain the privacy of protected health information, provide this notice, and follow the terms of the notice currently in effect.
